Medicare or Insurance Orders

Free shipping with a 90-day supply order (supplier must have on file a current Physician Prescription, and proper Medicare or insurance eligibility).

At this time, Four Leaf Clover, Inc. (FLC) currently ships orders via Priority Mail (standard rate only) within the 50 United States. Exceptions may incur an additional rate charge.

NOTE: Any order that does not conform to the regularly approved 90-day supply order program may be invoiced for the shipping rate charged to FLC.

Non-insurance Orders

All orders will be Priority mailed. An exception must be requested.

The costs of shipping will he shown on the supply invoice.

All Non-insurance Orders (cash) will be pre-paid prior to shipping.

NOTE:

At this time, once your order is received, processed and approved it will be shipped via Priority Mail and depending on the zone (distance from your supplier) received normally within three business days.

Return Policy

Diabetic supplies are prescribed for home care and cannot be re-dispensed. Therefore, these items cannot be returned for credit.
